Job Summary:

The Medical Social Worker is responsible for providing quality home health services to assigned patients under the direct supervision of the Clinical Supervisor. Through competent application of social work processes and use of accepted best practices, provides quality home health services to assigned patients under the clinical supervision of a Skilled Professional. Ensures compliance in accordance with federal, state, local regulations and agency policies and procedures.

Minimum Requirements:

  • Have completed a masters or doctoral degree in social work or related field.
  • One (1) year of clinical experience; home health experience preferred. Demonstrates knowledge of resources available within the community.
  • Current CPR certification.
  • Must have a valid in-state driver’s license, reliable transportation, provide current registration and insurance, and have a driving record with no more than two points within the last three years.
  • Ability to communicate effectively in writing and verbally with people who have varied backgrounds.
  • Demonstrate strong interpersonal skills and possess the ability to deal tactfully with personnel, patients, family members, visitors, government agencies/personal and the general public.
  • Be proficient in the use of computer software programs for internal and external communications and be capable of accurately documenting within the Electronic Health Record.
